Purpose. To estimate the prevalence and rates of progressive visual field loss in glaucoma patients followed annually for a median of 6.3 years. Methods. Linear regression was used to estimate rates of progression of MD, CPSD, clusters of locations based on the Glaucoma Hemifield Test (GHT), and location specific changes in C-30-2 fields of the Humphrey Analyzer. Results. Sixty-seven eyes of 56 subjects whose 1st two consecutive fields were abnormal on GHT were included. Visual field progressed in 19 (28%) eyes based on worsening of CPSD, GHT clusters or individual locations. 5 eyes had progression of CPSD, 17 eyes progressed in at least one GHT cluster, and 15 had one or more individual locations progress. For those whose field loss progressed, CPSD changed by 0.9 dB/year. GHT clusters declined by between 1.4 and 2.4 dB/year. Progression at individual locations ranged from 1.0 to 5.0 dB/year. Progressive visual field loss was not strongly correlated with thinning of the nerve fiber layer or optic disc changes during follow-up based on masked photo readings. Conclusions. Fewer than 1 in 4 glaucoma patients had any progressive field loss. Changes of less than 1 dB/year could not be detected with 7 fields done over 6 years. Larger changes at individual locations would need to occur before such changes could be detected statistically.
