Abstract
Since the discovery of insulin in the 1920s, significant progress has been made in understanding the pathogenesis of type 1 diabetes and developing sophisticated approaches to manage glucose homeostasis in patients. Key aspects of the disease pathogenesis, however, remain poorly understood, hindering the efforts to find a cure and prophylaxis for this debilitating condition. These efforts are further complicated by gaps in understanding the risk factors and difficulties in early detection. In this chapter, we will highlight the major outstanding questions regarding disease pathogenesis and dwell on lessons learned and the promise of finding solutions in the near future.
