The Role of Transportation in PACE

Abstract

PACE programs are responsible for a wide range of services, including transporting participants to/from the Day Health Center as well as to all healthcare related appointments. Drivers and escorts are typically matched in “teams” to increase efficiency and familiarity with participants and caregivers. In addition to providing a means from getting from one place to another, PACE transportation staff provide comfort and reassurance to the participants as well as valuable information to the interdisciplinary team to improve care and outcomes.
