The role of radiation therapy in clinical stage I carcinoma of the endometrium

Abstract

The survival experience of 321 patients with carcinoma of the endometrium is described. Of the 287 patients who had definitive surgery, 94% were clinical Stage 1; the proportion of patients having a more advanced pathologic stage as determined at surgery correlated with histology grade: grade 1, 3.5%; grade 2, 22% and grade 3, 55%. The 5 year survival results are analyzed according to clinical stage and treatment policy i.e. initial surgery with postoperative radiation reserved for selected patients vs preoperative radium or external beam therapy followed by surgery. The results indicate that for the operable patients whose disease is clinical Stage I with grade 1 histology, no preoperative radiation therapy is needed For the patient with grade 2 histology, the improved survival with preoperative radiation therapy was not statistically significant; however, the difference in recurrence rates was significant and preoperative therapy therefore is recommended. Preoperative radiation resulted in improved survival for clinical Stage I, grade 3 patients and its use is essential to provide these patients with the best chance of cure.
