CT densitometry and transthoracic biopsy are the most effective of new tools in the management of the patient with pulmonary nodules and allow prompt documentation of both malignant and benign lesions. Their proper place in the orderly diagnostic assessment of pulmonary nodules should be that of second line modalities following the appropriate use of radiography, including review of films, low kVp radiographs, and oblique views. A working understanding of the whole issue of pulmonary nodule management is helpful in determining the optimal diagnostic path to be followed in each case.
