Risk factors for choroidal neovascularization (CNV) in the second eye of patients with age-related macular degeneration (AMD)

Abstract

Purpose: To verify and quantify previously reported risk factors for development of CNV in fellow eyes of AMD patients with unilateral CNV. Methods: Follow-up study of fellow eyes of 670 patients enrolled in the MPS's clinical trials for juxtafovea! and subfoveal CNV secondary to AMD. Patient (N=670), central macular (N=485), and pericentral macular (N=333) characteristics at baseline were evaluated as risk factors for CNV development within 5 years through use of Kaplan-Meier estimation of cumulative incidence and application of the Cox model. Results: Four independent risk factors were identified: Factor Relative Risk (95% Confidence Interval) >5 drusen of any type (central) 2.1 (1.3, 3.5) Focal hyperpigmentation (central) 2.0 ( 1.4, 2.9) >1 large drusen (central) 1.5 (1.0,2.2) Systemic hypertension 1.7 (1.2,2.4) 5-year incidence rates ranged from 7% for those with 0 risk factors to 87% for those with all 4 factors. Conclusions: The prognosis of the fellow eye is affected strongly by characteristics of its central macula and by systemic hypertension. Patient counseling and frequency of follow-up tailored to degree of risk.
