Abstract
Purpose: To determine the amount of change in refractive error (spherical equivalent) over time in eyes with subfoveal choroidal neovascularization (CNV) secondary to age-related macular degeneration (AMD). Methods: In the Macular Photocoagulation Study (MPS) clinical trials for new and recurrent subfoveal CNV, standardized refractions were performed on study eyes (SE) and fellow-eyes (FE) at baseline (BL), 3 months, 6 months, and at 6-month intervals for up to 5 years. Distributions from BL, 1 year, and 4 years of 486 phakic study eyes randomized to laser treatment or observation, and the 192 fellow-eyes that never developed CNV were examined. The effects of new or recurrent CNV and of laser treatment were evaluated. Preliminary Results: (all values in diopters) BL 1-year 4-years Group N avg avg chg % chg ≥1 avg chg % chg ≥1 SE 486 1.3 1.5 +0.22 28 1.3 -0.04 40 Trt SE 234 1.5 1.5 +0.03 27 1.2 -0.30 41 Obs SE 252 1.1 1.6 +0.40 29 1.4 +0.20 40 FE 192 1.1 1.1 +0.04 8 1.1 +0.07 20 Conclusions: Within the group of study eyes, there were statistically significant changes observed from BL to 1 year, and between treated and observed eyes. The group of fellow eyes did not demonstrate significant changes over time. Additionally a large number of individuals had ≥ 1 diopter changes in their measurements over time. These findings demonstrate the need for standardized refractions in future prospective studies of subfoveal CNV in AMD.
