Posterior capsular opacification following extracapsular extraction for radiation induced cataracts

Purpose. To determine if extracapsular lens extraction of radiation-induced cataracts may be associated with lower rates of subsequent posterior capsular opacification. Methods. We perfomed a retrospective analysis of patients treated with extracapsular cataract extraction with or without IOL implantation. Inclusion criteria were: Age below 8 yrs, intact posterior capsule at end of surgery, and follow up of > 2 yrs. Exclusion criteria: vitreous loss, incomplete follow up, associated anterior segment pathology requiring simultaneous surgery. Patients were divided into two groups: Group I underwent radiation to the eye prior to surgery for the treatment of retinoblastoma, rhabdomyosarcoma, and aplastic anemia (radiation induced, n=13). Group II (n=6) had cataracts which were either congenital or developmental with no prior radiation, and served as controls. Results. Average age was 2.9 and 4.4 years for groups I and II, respectively. The time interval between radiation and cataract surgery was 31.5 months (group I). The mean follow up was 4.6 and 2.5 years for groups I and II respectively. Posterior capsular opacification developed in 7/13 and 3/6 in groups I and II respectively. The average time of development of visually- significant capsular opacification in group I was 27 months, which was significantly greater (p< 0.1)than group II (4 months). Conclusions. The development of posterior capsular opacification is delayed following radiation-induced cataracts. Longer-term results and additional data will be presented.
