N2 - To the Editor: Free air under the diaphragm on x-ray examination usually means rupture of a hollow viscus, and therefore represents a surgical emergency. An exception to this is exemplified by the case described below, in which a carefully taken sexual history revealed the cause, and surgical intervention was unnecessary. A 20-year-old married woman was admitted to the emergency room with a history of abdominal pain of two hours' duration. The pain had been of sudden onset, and she had been previously entirely well. The temperature was 36.7°C, the pulse 80, and the respirations 20; the blood pressure was. . .
