Patient safety: An overview

The increasing recognition that, despite spectacular progress in treating disease and improving health, health care can be unsafe for patients has triggered a considerable reassessment of contemporary medical practice in the context of a complex and opaque system. The science of human error and theoretical concepts from other safety-critical industries are increasingly relevant to the emerging interprofessional discipline of Patient Safety. Many challenges remain ahead to achieve safety and high-reliability from our health care delivery system.
