Abstract
This chapter outlines the use of deep brain stimulation (DBS) in the treatment of the three major neurological conditions it is used for, namely Parkinson's disease, essential tremor, and dystonia. Descriptions of the clinical indications, the procedure itself (including the use of the stereotactic frame system), and subsequent postoperative follow-up are provided in the chapter. Targets for treating the motor symptoms of idiopathic Parkinson's disease (IPD) include the globus pallidus pars interna (GPi) and the subthalamic nucleus (STN). Stimulation of either site produces durable improvements in the Unified Parkinson's Disease Rating Scale (UPDRS) and other functional scores with bilateral placement. In addition, the chapter describes new investigational studies in Tourette syndrome and epilepsy. It also discusses the electrode placement techniques via MRI guidance and microelectrode recording.
