A patient is presented with normal coronary arteries in whom total occlusions in three branches of two epicardial coronary arteries developed in association with chest pain and electrocardiographic evidence of transmural ischemia. A combination treatment strategy addressing both coronary vasoconstriction and thrombosis is illustrated.
