N2 - PURPOSE: To develop a research instrument for measuring effects of glaucoma and its treatment on quality of life and functional status. METHODS: We conducted 5 focus groups with glaucoma patients to identify specific quality of life issues related to glaucoma and its treatment. Transcripts were analyzed and 42 items endorsed by 2 or more participants were included in our trial instrument, which was mailed to 2,000 randomly selected subscribers to GLEAMS, the newsletter of the Glaucoma Research Foundation. We also surveyed demographic data, glaucoma status, medications taken, and comorbidities. Factor analysis was used to identify subscales and to select items for the final instrument. RESULTS: Of 2,000 surveyed, 1,361 (68%) persons responded. Of those, 91% had glaucoma, while the remainder were either pharmacists, relatives, or friends of glaucoma patients. Of the 42 items, a 31 item Glaucoma Disability Index (GDI) was constructed, Cronbach's α was 0.92 for the questionnaire and 0.44-0.92 for subscales. Higher GDI score (more disability) was associated with female sex (P
