note = "Funding Information: As for opium and other IRD, the world literature includes community surveys completed by the Indian Hemp Commission, and by E.G. Eberle in the United States, during the late 19th century. Scholars interested in opium, heroin, and opioids generally will benefit from a reading of the classic text entitled {\textquotedblleft}The Opium Problem,{\textquotedblright} authored by C.E. Terry and Mildred Pellens in 1928, with funding from the Rockefeller Foundation and the Bureau of Social Hygiene. Isidore Chein and colleagues published {\textquotedblleft}The Road to H{\textquotedblright} (Basic Books, 1964) as a summary of their epidemiological research on youthful heroin use in the 1950s, and in later work Carl Chambers and John Ball offered a mid-20th century compendium of later evidence on opioids in their edited volume titled {\textquotedblleft}The Epidemiology of Opiate Addiction in the United States{\textquotedblright} (C.C. Thomas, 1970). Funding Information: The work was supported by US NIH grant award K05DA15799.",
