Abstract

Hospital at Home (HaH) provides care usually performed in the traditional acute inpatient hospital setting in a patient’s home. In this chapter, we describe the problems addressed by HaH, define the model and how it works, provide an overview of the supporting evidence base, and explore key issues related to the implementation and dissemination of HaH and how HaH could anchor a future home-based care ecosystem.
