Abstract
Laparoscopic surgery has been rapidly developed thanks to the advantages that it gives compared with laparotomic surgery. This approach can lead to problems that are given by pneumoperitoneum: gas insufflation and cardiopulmonary side effects on patients. For this reason we have tried to obtain a working cavity room lifting the abdominal wall without the aid of CO2 gas insufflation. To this purpose the Authors developed a V-shaped elevator that is placed in the abdominal cavity, raised and fixed to a suspension system. The working cavity can be increased by the traction of sutures on the abdominal wall. 22 patients have been submitted to this technique: 20 patients underwent to gasless laparoscopic colecistectomy and 2 patients underwent to laparoscopic gynecologic surgery. In only one case traditional laparoscopic surgery has been required. This technique is less expensive than the gas technique because there is not the problem of the pneumoperitoneum maintenance so that non-disposable trocars and ordinary surgical instruments can be used. Post-operative results of gasless laparoscopic surgery are comparable to traditional laparoscopic surgery ones.
