Gallstone ileus presenting as small bowel obstruction: Spiral CT evaluation

Gallstone ileus is the result of a sequence of events that are initiated in the gallbladder. Inflammation of the gallbladder wall allows erosion of a gallstone through the wall into the small bowel and subsequent bowel obstruction. We present a unique case in which CT scans done over a period of 1 year demonstrate this progression in a patient and alter the clinical management by identifying the correct cause of small bowel obstruction. The clinical presentation, radiologic findings, and management of gallstone ileus are reviewed.
