Carbon monoxide poisoning due to automobile exposure: Disparity between carboxyhemoglobin levels and symptoms of victims

Abstract

The case of this family provides several teaching points. (1) The levels of carboxyhemoglobin can be dissimilar in individuals simultaneously exposed to the same source. (2) The symptoms manifested by the individuals can be dissimilar despite similar carboxyhemoglobin levels. (3) Levels of carboxyhemoglobin present in individuals riding in old vehicles can be as great or greater than those of individuals who are victims of house fires. Whenever CO poisoning is suspected, prompt initiation of 100% O2 therapy is recommended. In patients with serious clinical findings or with carboxyhemoglobin levels > 20% to 25% and in women who are pregnant, hyperbaric O2 therapy is recommended.
