Abstract
According to Ficat and Arlet excellent results were achieved when central marrow decompression was carried out prior to the appearance of the radiological features of pathological change or breaching of the hip bone. After collapse of te hip bone but before rupture, marrow decompression has a palliative effect although there is a gradual increase in clinical signs as the postoperative time period increases. The authors' experience corroborates these findings and in their opinion the key to success lies in early detection. All these findings indicate that marrow decompression is the treatment of choice in ischemic hip necrosis in the preradiological stage and before the femoral head has collapsed.
