Abstract
Glaucoma surgical choices are continuing to evolve toward safer options as a first-line therapy. MIGS procedures open the door for the patient who you might want to avoid filtration procedures. Avoiding blebs, flat chambers, and choroidal hemorrhage is an issue that made surgeons resistant to consider glaucoma surgery in the past. The surgery choice is based on the desired risk-to-benefit ratio. Avoiding a flat anterior chamber or bleb may be outweighed by the need to preserve the optic nerve. Where to use new procedures and how much risk to expose you and the patient to leads the surgeon to the new procedures considered.
